Most missed putts aren't a bad read. They're a good read with a face that wasn't actually square to it at address.
Human eyes are notoriously bad at judging square from behind a putter — you'll often set up with the face a couple degrees open or closed without ever knowing it. That small a miss, over a ten-foot putt, is the difference between center-cut and lipping out.
A cheap alignment aid or even just laying a shaft on the ground during practice will show you a gap between what you think is square and what actually is.
